Clinical Practice and Partnerships
Field Experiences are vital to teacher preparation at Delaware State University, as they allow students to practice in the field what is learned in the classroom. Students who participate in these experiences are required to maintain updated criminal background checks, TB Test, Child Protection Registry clearances on file in the Office of Clinical Practice & Partnerships.
The Requirements that follow are to be obtained at the expense of each education student. Please ensure that you carefully follow the directions. You will not be eligible to receive a field placement if the required documents have not been turned in by Wednesday January 15, 2025 and you will have to drop the course prior to the drop/add deadline. Failure to complete the field experience will result in a grade no higher than a D and the course must be repeated.
- Complete the Criminal History Record Check Form. Background checks must be done once a year and you must have an appointment. To schedule your 10 minute fingerprint appointment for the background screening, simply visit https://uenroll.identogo.com and enter the following Service Code 27S2Q7 You must bring one form of identification to your appointment. This identification must be valid, not expired and contain your photograph. If you do not have access to the internet to schedule your appointment you can still schedule by calling 866-761-8069. The cost for the background check is $38. Please send a picture of your receipt to the secretary, April Daigrepont.

Complete the TB Test Form. The TB test can be obtained from your Primary Care Physician or any medical lab such as Med Express, 15 South DuPont Highway, Dover, DE 19904; 302.674.1514. The fee is $41. The test is a 2-day process. The initial visit is to receive the shot and the second visit is to have the reading. The reading must be done within 48-72 hours of the initial visit, or the test is invalid, and you will have to repeat the process for an additional fee. You may bring the completed and signed form from a doctor’s office and give it to the secretary, April Daigrepont.

Complete the Child Protection Registry Form. This form is to be submitted online at https://childprotectionregistry.delaware.gov. Complete the form and save to your computer. Click on “New Individual Registration” and register. You will receive an email and you will be given a link to change your password. Then click on Create New CPR Request, fill out & click on “confirm”. Click link to upload consent form, review & save. Make payment of $14 and submit. Send picture of receipt to the secretary, April Daigrepont.
